Scheduling an Appointment:
- Click on the Book Appointment tab
- Schedule your in person or telemedicine appointment
New Patient Appointments:
- Typically 45-60 min
- Can be in person or by telemedicine
- Join your portal and complete the intake before your appointment.
Free Consultation or Assessment:
- 15 min consultations can be in person or by telemedicine
- 30 min assessment is in person
When You Arrive:
- We are in The Health Collective building on the corner of 5th.
- Park in front or in the lot off of 5th.
- We are on the first floor.
- Come in and have a seat and water or tea.
